UMR’s Plan Advisor phone line is available to help with locating and confirming medical providers that are in your health plan’s network. You will need to provide your social security number to identify yourself when asked for your Member ID number.
When calling Plan Advisor, select the following prompts:
- Member
- Medical
- Provide your SSN as your Member ID

Confirm Your Doctors are In-Network
The healthcare provider network for members that reside in New York State is Choice Plus with POMCO Select.
The healthcare provider network for members that reside outside New York State is UHC Choice Plus.
We encourage you to confirm that your preferred health care provider is in this network. You can do this at the UMR website, www.umr.com/find-a-provider.
ID Cards
All members will receive a new ID card directly from UMR. These were mailed in June.
